disclose this information, even when our customer relationships end,

Information We Collect
PFB Members’ Service
Corporation, Health Services Division, collects and retains nonpublic
personal / financial information about you that includes your name,
address, Social Security number and in some cases income tax and financial
information. We receive this information from the following sources:
Membership applications, health insurance enrollment applications and
other underwriting forms provided to us by the member; Transactions
such as premium payments and claims submissions that are mailed directly
to us by the member. Outside sources, such as health care providers,
other insurance companies and federal and state agencies. Nonpublic
personal information is not disclosed to third parties except as necessary
in performing our services to you or as required or permitted by law.
Access to nonpublic personal information is restricted to authorized
employees and staff, and is used only in the course of providing the
services you have selected.
How We Protect Member Information
PFB Members’ Service
Corporation, Health Services Division, maintains physical, electronic
and procedural safeguards that promote compliance with this policy.
In addition our health insurance carriers have disclosure practices
that are in compliance with this law. All Farm Bureau members participating
in our group health care program receives their annual Notice Of Privacy
Practices.
